PI: Elizabeth Gyllstrom, PhD, MPH; Co-Investigators Susan J. Zahner, DrPH, RN, FAAN and David Hahn, MD

Funder: Robert Wood Johnson Foundation ($450,000)

The purpose of this project was to study the variations of primary care (PC) and public health (PH) integration. This multi-state project included PC and PH practice-based research networks from the states of Minnesota, Colorado, Washington, and Wisconsin. Research questions included: 1) How does the degree of integration between PC and PH vary across local jurisdictions? 2) What factors appear to facilitate or inhibit integration, and how can PC and PH leverage those factors to increase integration? 3) Does the degree of integration differ based on health topic area? and 4) Do areas of greater integration have better health outcomes?
